Below you will find a sample of the gift voucher that will be given to your assigned loved one. However, if you would like to give a fixed amount, or a particular treatment, we can customize the voucher according to your liking.

Terms & Conditions:
Vouchers are valid for 6 months from the date of issue and must be booked and redeemed before the expiry date. It is the voucher holder's responsibility to make a booking in sufficient time before expiry. Difficulty finding a preferred appointment time does not automatically extend the validity period. If no suitable availability is showing online, please contact Wellness Revived before the voucher expiry date so that we can assist with arranging an appointment.
